The botched execution of Christa Pike at Riverbend Maximum Security Institution, the second failed execution attempt this year, is why Tennessee must do more than review what went wrong.
Yet Gov. Bill Lee did just that hours after Pike survived two lethal pentobarbital doses before being treated at a hospital for undisclosed injuries. The doses were supposed to stop her heart, but didn't. As witnesses were describing what they saw and heard, emergency vehicles rushed onto the prison site.
